Upon receipt of the completed form your policy will be cancelled as of 12/1/2014 12:00 AM. All future and prior acts coverage will cease under the policy as of the cancellation date. Please be aware that if you plan to continue a career in the insurance industry most claims-made E&O policies do not provide prior acts coverage when continuous coverage is not maintained. Completed cancellation request forms can be faxed to 1-800-411-4771.
