Formal and informal assessments indicate SEV Receptive/Expressive language disorder and Severe Pragmatic skills. However, speech-language services are recommended to increase his Speech and Language Skills to a developmentally appropriate level to facilitate communication between the patient, his family, and peers. Prognosis is judged to be good at this time based on excellent family support.

1-Decreased comprehension and reasoning skills for age and level of cognitive function.
2- Decreased auditory memory for details in information heard given his level of cognitive functioning.
3- Inability to maintain a conversation on the topic with peers, family, and teachers.
4- Impaired verbal narrative skills for age and level of cognitive function.
5- Impaired speech intelligibility in conversation with family, adults, and peers.

The SLP recommended a Home Program designed for his family to continue working with Pt's communications skills in daily living and educational performances. The Clinician will follow up with the caregiver to monitor the process and generalize skills into daily living.
