Present: Dr. B, Jane Vale MDS, Lindsay Lyon SS, Diane Sewell RN DON, Jacque Marchbanks Admission, Cindy Foster MR, Shirley Porter DM, Jennifer piatt, Cindy Foster MR,

Old business: New DON , Diane Sewell, Jennifer Piatt SDC, looking for SS director, Lindsay taking certification classes, , Jodi COTA

New Business: Started Cert classes, interviewed 3 SSD's, Darcie Johnston, coming Monday for 2nd interview.
Unit to be locked all paperwork done, waiting for lock, Life Safety needs, need Director and State needs to approve. Dementia Experience 4-21-15, all community invited.,

Paperless , scanners, PCC training, discussed QHN. Continue to work on integration.
Restorative Program started last week, Communication, ADL, Ambulation, Safety, Reminiscing,maintanience care to prevent decline.

Admissions: Referral from MMH, G.J. Community St. Mary's, Kindred , SNF's, and AL. Only 1 from Delta. @ MD referrals. Reasons, families local, holding area, respite, mostly Medicaid, Dr. Settles comes into building on every Thursday and stops and sees is residents. Many admissions, Delta hospital is slow. Decreased amount of rehab patients and ARU have had decreased patients. We have received from PACE due to dis-enrolling.

Chart reviews: Cindy: New admission charts only, 10 to close
QI: 8 infections; 3 Resp, 2 UTI, 1 skin , 1 oral, 1 sinus
P.U. 3 in March all admitted with.
Physical Devices: none, wander
Weight Loss: Susan Likens LCSW visits weekly, able to state needs, 10% weight loss, ( Beverly Carter), comfort measures, DNR. Will request family conference, family has been called twice with no response.
Falls: 25 falls for March, 2 with serious injury; 3 repeaters
Depression: screening questions may be squewed... PHQ9 may do but not be able to do a BIMS as accurately
Catheters: 3
Labs: Pending creation of lap protocol. standing orders. Dr. B will continue to work on a system.
Discharge Planning: Worley into AL in Delta, Herrara to community, Peter to community, Tim to community.
These resident do not currently have homes and may see as new trend. AAA Transitions program has been established. Residents are not performing follow through with filling out medicaid applications for county and or resist assistance from staff. Have to know Life Insurance Policies and all resources. Family/resident concerns: repeat behaviors with another resident on the hall and have allowed facility to take to Dr. Good, appointment is asked for and pending.
Advance Directives: 2 sent in to get signed by MD. Copies are in charts.
Dietary: MOD working well. Fresh plant or flowers for each table. Waiting for new freezer.
Safety/Health: Meetings being held, saved cost containment for plan. Review recently for fire
Pain management: 2 for severe pain. Tim, Worrley
Investigations: none at this time
State Survey: action plans, risk management, out to state in march with an action plans in place
Education: lots!
