If you raise 5x BB on a good hand, 3x BB (to slowplay) on AA and KK and 6x BB when you're bluffing, an astute player will be able to read you very quickly.
A continuation bet is a bet you place on the flop after making a raise before the flop. It doesn't have to be a bet with a real hand. It's just any bet you make after a preflop raise. The term has a connotation that associates it with bluffing because most hands with which you raise miss the flop completely. In other words, most continuation bets are bluffs.
Your opponent's chip stack also makes a huge difference, not only to the size of your bet, but as to whether or not you will bet at all. If you make a pre-flop raise of $300 and your opponent calls with only $300 left in his stack, he is pot-committed and you will not be able to bluff him after the flop. Realize this and either flop or be prepared to put him all in.
In reality, "clever" plays should be used only every once in a while. It shouldn't be the norm. In general, bet the value of your hand. If visit here have trips, bet out. If you have nothing and your opponent bets, fold.
