2. Thank you for taking my call, we're on a recorded line for quality purposes. The reason we're calling is we want to thank you for choosing and maintaining your Citibank credit card. CFSI, an affiliate of Citibank has entered into a partnership with BPLAC to provide you a protection against critical illness.

3. To confirm, base po sa aming record kayo po ay ____ y/o? Married na po ba or single? I'll ask you 3 health questions if you're qualified for the critical illness plan. (**3 health questions. And if lead responded 3 NO's, proceed**) Thank you for your honest answers and you are eligible for the Critical Life Saver Plan.

4. And to also share with you, si BPLAC pa lang po ang nag o-offer ng insurance na kung saan ay Early Stage Diagnosis pa lang po ay covered na kayo. This means po na if madiagnosed kayo ng Carcinoma-in-situ or Stage 0 cancer or kahit simpleng bukol lang po and nabiopsy and it turned out po na naging Early Stage Malignancy ay covered na po kayo.

5. Ang covered po ng Critical Life Saver ay CANCER, HEART ATTACK & STROKE. Sa Early Stage Condition naman po na covered ay Carcinoma-in-situ, Early Stage Malignancy, Cerebral Shunt Insertion, Endovascular Treatment for Cerebral Aneurysm, Less Severe Heart Disease, Minimally Invasive Direct Coronary Artery Bypass and Less Invasive Treatments of Heart Disease.

6. Critical Life Saver Plan have 4 benefits. Sa Early Stage Critical Illness Benefit, you'll receive 25% of the sum assured if madiagnose po kayo ng early stage [Plan 500k: 125k. Plan 1M: 250k]. Sa Medical Treatment Benefit, you'll receive 10% of the sum assured [Plan 500k: 50k. Plan 1M: 100k] in case po na magrequire ang inyong doctor na kailangan po ninyong mag undergo ng Surgery, Chemotherapy/Radiotherapy, Rehabilitation & Medication. Sa Special Compassionate Benefit naman po may marereceive po ang inyong beneficiaries ng 5% of the sum assured [Plan 500k: 25k. Plan 1M: 50k]. And sa Critical Illness Benefit naman po may marereceive kayo na 100% of the sum assured once po na madiagnose ng late stage.

7. Once po na maclaim na ang 500k or 1M na sum assured, stop na po ang monthly premium and coverage ninyo. Payable once po ito and ang magiging monthly premium nyo po ay Php_____ for the coverage of 500k/1M. Mag set aside lang po kayo sa budget ninyo ng Php____ everyday and that is for 5 years. 5 years renewable term po ito and for 5 years, hindi po mababago ang inyong monthly premium, magbabago lang po ang monthly premium ninyo once po na magrenew na kayo.

8. 2 policy po ang inyong marereceive, isa pong soft copy sa inyong active na email address at isang hard copy sa inyong preferred na billing address kung sa home or office address at iyon po ang katibayan na nakapag enroll po kayo sa plan ni BPLAC. And very convenient po ito para sa inyo since hindi na kayo magfifill out ng form and no need for physical and medical requirements. And for the charging/payment naman po, auto-debit na po ito sa inyong Citibank Credit Card.

9. Lahat po ng nadiscuss ko sa inyo ay nakaindicate sa inyong policy na marereceive nyo po within 30 days upon successful billing of your monthly premium. To process your enrollment, Ask ko na lang po kung san nyo po prefer na mareceive ang inyong policy? Sa home address po ba or sa office address? Kindly state na lang po over the recorded line.

11. Thank you for availing BPLAC Critical Life Saver! Your coverage will take effect upon successful billing of your initial MONTHLY premium on your nominated CItibank Card. You'll be notified on the status of your application by BPLAC.

And your BPLAC Critical Life Saver Policy will be delivered to your preferred mailing address within 30 days upon successful billing. Upon receiving the policy contract, you may refer to the document for full details on the critical illness definitions, exclusions including the pre-existing conditions and other limitations. If you decide that this plan is not suitable to your needs after reading the policy contract, simply return the document to BPLAC within 15 days from the date when you or your authorized representative received it and we will refund to you the total premiums you have paid.

Your benefit under this policy will be paid directly to you. In the event of death, the benefit will be paid to your first surviving class of the following beneficiaries.
