Complete Social Security Number

· If the claimant does not provide the complete Social Security number, they must provide their claimant ID and the last 4 digits of their Social Security number.

Complete Name including First Name, Last Name, Middle initial, Suffixes (Sr, Jr, I, II, III) if any

· Can say “Can I have your full name including middle initial if

· Can say “May I please have your full name as it appears in Connect with any middle initial, middle name or suffix you may have listed

Complete Date of Birth (month/date/year)

· Can say “May I have your DOB in the following format, Month, Day and Year

Complete Mailing Address including/ Apt/Unit/Lot/St/Ct/Cir. . .etc.)

· Can say “Is it a house?” or “Please provide the address as it appears on your DL or State ID.”

· “May I please have your complete address? “

· If the claimant failed to verify information such as St, Ave, Ct, Blvd, Rd, Hwy, Bldg. #, Apt, Unit, Suite, Lot #, Hotel room # etc., you, the agent should probe without releasing any claim or claimant information to the caller.

· If the claimant only mentions # 105 and, on the claim, it shows APT # 105, the agent should ask the claimant to clarify what the # is

· If Claimant is not able to verify telephone number or if no telephone number is available in CONNECT, the Agent must verify one of the following:

o DL/ID# (Authentication)

o Employer on record (Authentication)

o Email as it appears in Connect (Password & Contact)
