Presented with history of worsening shortness of breath for the last 3 days associated with poor oral intake and nausea family took the patient recently to investigated the poor oral intake and had US abdomen done, which incidentally showed moderate amount of right sided pleural effusion patient id being treated for pulmonary fibrosis with nitidinib prior to admission.
He was found to be positive for adenocarcinoma in pleural fluid on the right side. he had a pigtail placed for drainage gradually to avoid lung edema upon re-expansion. The pigtail catheter drained total 1500ml over the period range of 5 days. It was removed on 21st of November by patient request
The patient's transfer has held due to covid positive statues that is now cured with favipiravir and dexamethasone course of ten days, two consecutive negative on 29/11/2020 and 30/11/2020
Now lung is building up fluids again with loculated nature and family refusing further care given two negative covid test and requesting transferee to a different facility
