S: Consent gained, Post fall review. Donal uses bed rails and crash mattress. Donal climbed over the rails and was found sitting beside his bed. Had past h/o right femur # and surgical correction. Not in any pain, had small bruise on head after the fall .Uses hoist and Ax2 for all transfers. CNM DOM and S/N Kay present for the assessment. O: In bed ,rails up and crash mattress. Good functional strength and ROM in Ul and LL. Lying to sitting min Ax1. Sit to stand WZF and Ax1. Right hip goes to adduction while mobilising in wzf, but able to correct with prompts.Good sitting posture in chair. Not the proper footwear,uses slipper and not great grip. A: Donal is well able to get out of bed with min Ax1 ,and do sit to stand with X1 IN WZF.Donal has good potential for improving his gait and mobility with practice. P: 1) wedges, floor mat ,crash mattress and low low bed position. Bed rails dowm. 2)Transfers in wzf with Ax2. 3) Mobilise in wzf with Ax2 from room to lounge ,would need prompts for proper foot positioning. 4) sitting out in chair with supervision. 5) Review on next visit to progress mobility as able.
