• Hasta Haklan Yönetmeliğinin 24. Maddesi "Tibbi müdahalelerde hastanın rızası gerekir. Hasta küçük veya
mahcur ise velisinden veya vasisinden izin alınır" • Hasta Haklan Yönetmeliğinin 25. Maddesi "Kanunen zorunlu haller dışında ve doğabilecek olumsuz sonuçların sorumluluğu hastaya ait olmak üzere; hasta kendisine uygulanması planlanan veya uygulanmakta olan tedaviyi reddetmek veya durdurulmasını istemek hakkına sahiptir."
Yukarıda sıralamış olduğum maddeler gereğince çocuğuma yapılacak olan hiçbir tibbi müdahaleye, hiçbir "SAĞLIK TARAMASI"na ( sürüntü testi, ilaç tedavisi, aşı, maske ve sair) ve ikram olarak sunulacak herhangi bir yiyecek, meşrubat, süt ve benzeri ürünlerin verilmesine, yedirilmesine ASLA RIZAM YOKTUR, İZİN VERMİYORUM.
Konunun öğretmenlerine ve okula herhangi bir sebeple gelecek sağlık ekiplerine bildirilmesini
istiyorum.
Rızamız ve talebimiz dışında çocuğumuza herhangi bir tibbi müdahalede bulunulmasının
• TCK'nin 86'2-3-b,c,d maddelerince "kasten yaralama
• TCK'nin 89. Maddesinde belirtilen "taksirle yaralama"
TCK'nin 257. Maddesindeki "görevi kötüye kullanma"
Ve ilgili başkaca suçları oluşturacağını hatırlatır, böyle bir durumda konuyu tüm hakkımı saklı tutarak Cumhuriyet Başsavcılığına intikal ettireceğimi belirtmek isterim.
