<p class="p__0">Karen Broadbent Michael Lovegrove A male, 26 years of age, presented to a hospital emergency department suffering a sudden start of an aching throat 6 hours formerly while consuming a carbonated beverage at work. The pain commenced about lunchtime, after he had been mixing concrete powder, and since then had been periodic and was becoming more severe.</p>

<p class="p__1">He denied any nausea, vomiting or shortness of breath but had severe pain on swallowing. He had no relevant previous case history, was not takingany medications and had no allergies. Observations were average with temperature level 36. 6C, pulse 54 bpm and regular, pulse oximetry 99% on room air, and respirations 16 breaths/min with equivalent air entry on both sides.</p>

<p class="p__2">On palpation he had tenderness in the anterior part of his neck, but no tenderness in the supraclavicular region. There was no subcutaneous emphysema. A regular electrocardiogram showed typical sinus rhythm and no severe ischaemic modifications and bloods were taken for a troponin level, which was typical. While recognising that the most likely medical diagnosis was pharyngitis, the emergency situation nurse practitioner was concerned that his signs were more serious than anticipated for such minor clinical findings.</p>
