35-year-old patient, mother of two children. She lives in London. In 2011 polytrauma from
road investment while crossing the street in London. She was following a long hospitalization for
polytrauma with cerebral contusions affecting the right hemisphere with residual hemiparesis
left and left hemihypoesthesia which still remain. Subsequent appearance of sensation of
subjective vertigo with a feeling of depersonalization, this symptomatology is short-lived
duration followed by fatigue, tendency to worsen over time and for about a year
associates that she is unable to control subsidence of the right lower limb, sometimes she signals
also feeling of making in the breath. In addition, a migraine has appeared since the accident
chronic for which she was treated with amitriptyline and topiramate with little benefit.
She had a brain and cervical MRI showing cortical atrophy at the left oparietal site
and partly to the occipital lobe, cervical cord free, disc protrusion c3-c4 a
right with electromyographic evidence of radiculopathy with signs of neurogenic suffering
chronic in territory c5-c6 c7. she recently underwent another brain MRI in Italy that in
June 2021 which confirms the presence of post-traumatic outcomes affecting equal weight and occiput
right polar with atrophy and gliosis at the edge of the field spondylouncoarthrosis was reported at
seat c3-c4- c4-c5- and c5 c6.
she also performed an EEG study which did not detect intercritical or parosistic abnormalities
criticisms. this investigation had been made on the assumption that episodes of malaise with sensation
of depersonalization and inability to control the lower right limb could have an origin
epileptic. such incidents in London were attributed to "functional" causes such as crisis
"piscogene" and indicated a neuropsychiatric type path.
