Etiyoloji
Sirozun etiyolojik nedenleri toplumun kültürel ve ekonomik yapısına göre farklılık göster
mektedir. Kuzey Avrupa ve Amerika gibi gelişmiş ülkelerde sirozun en önemli nedeni alkoliken,
Asya ve Afrika gibi gelişmemiş olkelerde sirozun en önemli iki nedeni Hepatit B veya C'ye bağh
viral hepatitlerdir. 5 yıl boyunca günlük 160 gr ya da 10 yıl 40-50 gr alkol kullanımı karaciğerde
yağlanmaya neden olarak yıllar sonra karaciğer sirozunun gelişme riskini arttırdığı bildirilmiştir.
Hepatit B'ye bağlı siroz 5-10 yıl içerisinde gelişirken, Hepatit C'ye bağlı sirozun gelişiminin 30
yıl aldığı gösterilmiştir
. Kronik Hepatit C taşıyıcısı olan hastaların %25'inde siroz gelişmektedir.
Olkemizde sirozun en önemli nedenleri ise kronik viral hepatitler (Hepatit B veya C) ve alkoldür.
Ayrica küçük ve orta boy intrahepatik safra yollarının ilerleyici harabiyetine bağlı da primer bil-
yer siroz tanısı konabilir. Hemokromatozda karaciğerde aşırı demir birikimi, Wilson Hastalığın-
a da bakır birikimi gibi metal ve metabolitlerin karaciğerde birikimi sonucunda 10 yıl sonrası
roz gelişebilir. Sağ kalp yetersizliği, trikuspid kapak hastalığı, perikardit, miyopati karaciğerde
njesyonun artmasına neden olup asit oluşumuna ve portal hipertansiyonunun gelişmesine
den olarak kardiyak sirozun gelişmesine yol açar. Bazı hastalarda etiyolojik neden saptanama-
bilir, bu siroz tipi Idyopatik ya da kriptojenik siroz olarak tanımlanmaktadır.
