Hepatik Ensefalopatide Tedavi ve Bakım

Azotlu bileşiklerin yapımına neden olan maddeler azaltılmalı veya elimine edilmeli,
Eğer koma kanamaya bağlı olarak meydana geldi ise, kanamayı değerlendirmek
kanamayı mideden uzaklaştırmak için mide lavajı yapılmali,
Gastrointestinal kanama veya kabızlıkta ortaya çıkan hepatik ensefalopati esnasında
hastalara lavman yapılmalı,
Diyette protein kısıtlanmalı; eğer hasta komada ise protein tamamen kısıtlanmalı, ko-
maya eğilimli bir hasta ise günde 40-60 gr'a kadar protein kısıtlanmalıdır.
Kolon bakterileri azaltılmalı: Bu amaca yönelik hastaya oral yolla Streptomisin veya Neomi-
sin verilir ve böylece amonyak ve diğer azotlu bileşiklerin yapımından sorumlu olan bakte-
rilerin sayısı azaltılır.
Barsaktan amonyak emilimi önlenmeli: Bu amaca yönelik kolon bakterileri ile fermente olan
ve daha düşük dışkı pH'sına yol açan bir disakkarid olan Laktüloz kullanılır. Bu düşük pH
amonyağı yakalar ve yayılmayan amonyum iyonuna dönüştürür. Laktüloz uygulanan has-
ta diyare ve hiperglisemi açısında gözlenmeli ve bu ilaç kesinlikle Neomisin ile aynı anda
verilmemeli çünkü, Neomisin intestinal florayı yok eder, ancak Laktülozun etkisini göster-
mesi için bakteriye ihtiyaç vardır.
