1) Find out the subject, verb, object, place and time expression!
2) Take the object in active sentence and use it as subject in Passive Voice.
3) Decide if the object in Active sentence is singular or plural!
4) Write the Object in active sentence as Subject in Passive Voice and use be form (am/is/are/was/were/has been/have been ..etc.)
5) Take the verb and use past participle form of the verb!
6) Put "by" if it is necessary
7) Take the subject in Active Voice as an Object for Passive Voice!
8) Then use place or time expression.
ACTIVE : Mahmut prepared a breakfast for his girl friend aat home in the morning.
1) Subject : Mahmut
Verb: prepare (Past Simple)
Object : a breakfast
Place: at home
Time : in the morning
2) Breakfast for his girlfriend
3) Breakfast for his girlfirend (Singular)
4) Breakfast for his girlfirend was
5) Breakfast for his girlfriend was prepared
6)Breakfast for his girlfriend was prepared by
7) Breakfast for his girlfriend was prepared by Mahmut
8) Breakfast for his girlfriend was prepared by Mahmut at home in the morning.

PASSIVE VOICE ( BE (NOT)+ PAST PARTICPLE)

Subject : Damla
Verb: makes (PRESENT SIMPLE) -am/is/are (not)
Object : delicious cakes for me
Place: at home
Time : everyday
Delicious cakes for me are made by Damla at home everyday.
