I would recommend with the current situation to do an application as one of the following based on your situation and goals:
If you need financial aid you want to apply as a degree seeking student in your planned field of study (undergrad or grad based on your class selection).
If you don't need financial aid and it is just one or two classes you want to take, then you will want to apply as a non-degree seeking student (undergrad or grad based on your class selection). Non-degree seeking students will need to make sure to re-submit their applications each semester they decide to take classes.
With both of these you will be able to use your credits either to continue to take classes at UNO or to transfer to another program outside of the University.
If the classes are Graduate Level then you will need to meet the admissions requirements for the Graduate School as well seen in the following link; keep in mind each graduate school also has their own requirements as well:

Either way you will want to submit a new application, which will be easier than the blue form for now. Please let me know if you have any other questions.
