Thanks for providing us the requested test page, I have reviewed it and was able to see our tag fired. However, it appears the provided test page is just a preview from the google platform and seems not to justify the actual serving. Nevertheless, I noticed that in the provided preview our ad seems to be fired multiple times, see this screenshot:
Please do note that in-stream ads consist of trackers only and it is the publisher's responsibility to make sure that they are calling/firing our tag/tracker only if they are counting impression. I have a second look in our logs and noticed that there are repeated IP captured on our end which appears to be the reason for the discrepancy.
