Hair Extension Online Consultation Form

If you would like to book an appointment for the application of Hair Extensions please fill out the following form. We will assess your suitability for Hair Extensions and get back to you within one working day. If you wish to go ahead with the Hair Extension service you can check availability and book online using our online appointment diary system. We will require a deposit before we can confirm any appointments, this can be paid using your debit/credit card either via telephone or our online booking diary. Please answer the question as accurately as possible, if we find your hair to be in too poor condition when you arrive for fitting of the extensions, we will be unable to carry out the service.

Please note, we CANNOT fit hair extensions under the following circumstances:

*Clients who are pregnant *Clients who have had a baby in the past 6 mths * Clients suffering from hair fall conditions * Clients suffering from scalp conditions * Clients who's hair is not 3-4 inches in length at the shortest point.

Please note, deposits are non refundable.

I understand that correct Aftercare Products are essential and i am responsible for making sure these products and used at all times. I will not hold the stylist responsible for any damage or injury caused by me failing to carry out the information & instructions given to me or for supplying any incorrect information at any time. I understand that regular maintanance is required to keep my extensions at their best and removal must be carried out after 3 months professionally for any bonded system.
