Abdomen-distended,soft tender at RHC no Visceromegaly, percussion note dull with fluid thrill +
Chest: bibasal decreased breath sounds with crepts
CVS:S1+S2
CNS-conscious well oriented GCS 15/15 with no sensory/motor deficit
PROGRESS
the patient was admitted with above complaints from OPD.
her baseline labs showed anemia (hb-8) and raised creat of 6 and her renal replacement on which she was on was resumed and we planned for a dialysis with ultra filtration,which she received 2 session of with reduction of weight from}}}}}}}}
She has improved both symptomatically and clinically and now being discharged home on medication
