1. Get 2 Blue Forms and fill up all the details
-agenda
-venue
-time
-date
-etc.
2. Encircle one with 'S.O. File' and 'O.S.A. file' on the other
3. Complete the signatures (president, adviser, dean)
4. Pass the forms to OSA with complete attachments
-Blue Form
-List of Participants (L.O.P.)
-Other attachments if necessary (ex. program flow, budget schedule, etc.)
5. Go back to OSA and check if the activity is approved or not BEFORE the actual date of the activity
5.1. If Approved, Get the signed 'S.O. file' white form and attachments and pass it to the
secretary that is in-charged of the Green Forms
5.2. If Disapproved, Get the papers and notify the VP-Internal and VP-I Assistant immediately
NOTE:
-Pass the blue forms 3 days before the activity
-Always sign in the logbook every time you pass or claim forms
-Our constitution states that we are required 2 General Meetings per month. It is your duty to ensure that this will be followed.
