1. Get 2 White Forms and fill up all the details
-name of activity
-venue
-time
-date
-etc.
2. Encircle one with 'S.O. File' and 'O.S.A. file' on the other
3. Complete the signatures (president, adviser, dean)
4. Pass the forms to OSA with complete attachments
-White Form (S.O and O.S.A)
2 Copies of the ff: (1 for the S.O. and 1 for the O.S.A.)
-List of Participants (L.O.P.)
-Program Flow
-Budget Schedule
-Other attachments if necessary (ex. pink form, parent's permit, etc.)
*S.O. = Original Files, O.S.A. = Photocopy
5. Go back to OSA and check if the activity is approved or not BEFORE the actual date of the activity
5.1. If Approved, Get the signed 'S.O. file' white form and attachments and pass it to the
secretary that is in-charged of the Green Forms
5.2. If Disapproved, Get the papers and notify the VP-Internal and VP-I Assistant immediately
NOTE:
-Pass the white forms 1 week before the activity
-You are in-charge for printing the Attendance List (Tutorial Participants)
-Always sign in the logbook everytime you pass or claim forms
