1. Print out the Documentary Pictures of the activity
2. Photocopy the Blue/White 'S.O File' Form and its attachments including the documentary pictures
3. Get 2 Green Forms and fill up the summary details
4. Complete the signatures
5. Encircle one with 'S.O. File' and 'O.S.A. file' on the other
6. Attach the 'S.O.' Green form on top of the original 'S.O' White/Blue form and attachments
7. Attach the 'O.S.A.' Green form on top of the photocopied 'S.O' White/Blue form and attachments
8. Pass the complete forms to OSA
9. Go back to OSA and get the signed forms and pass it to the VP-Internal Assistant
NOTE:
-Pass the green form 3 days after the activity
-Get the form stamped and signed by the OSA working student if there are missing attachments.
This is to abide the deadline rule. We call this process as "claim".
-Always sign in the logbook every time you pass or claim forms
