A Primary Survey MUST be carried out on ALL patient’s. It is the first thing you do once you arrive on scene. There are 2 Surveys that will needed to be carried out (Primary & Secondary Survey).Obviously, before you begin any treatment you begin this survey and if they can hear you/responsive, you also state: “Hello, Sir/Madam, I’m [rank] [name], with the Ambulance Service. Can you tell me exactly what’s happened please?”
Once stated and understood the emergency/situation, then you will begin the Primary Survey. This is abbreviated to: DRcAcBCDE.
D - DANGER Check for danger.
- Ensure you will not put yourself or any colleagues (at risk) of danger.
Check the area and if there is a danger, attempted to remove it as soon as possible.
R - RESPONSE Check for patient response.
- Check if the patient can respond verbally.
Check if the patient responds to pressure (e.g push/pinch their shoulder or squeeze their finger).
C - CATASTROPHIC HAEMORRHAGE Check for bleeding.
- Look at the patient and check for any obvious bleeding.
- Internal bleeding is a possibility and therefore check or think of any causes which may have caused it.
A - AIRWAYS Check for blockages in the airway.
- Check for any physical objects in the area.
Make sure the area is always open/secure, meaning that it has not collapsed or become smaller/closed up.
C - C-SPINE Check the cervical spine (C-Spine).
- In traumatic cases (e.g. RTC (Road Traffic Collision)) then you should always consider damage to the neck and spine. Failing to understand or observe this part of the survey could lead to paralysis if the patient has broken their neck or spine.

Gently feel the neck and spine-hips and check for fractured bones.
B - BREATHING Check for breathing.
- Check for the rise and fall of the chest.
Listen for breathing or attempt to “feel” the breathing (put your head next to their mouth and try to feel any air/breathing coming out onto the side of your face).
C - CIRCULATION Check for a pulse.
- Using your index finger, middle finger and ring finger to check for a pulse.
The most common places to check are the radial (wrist) pulse and the carotid (neck) pulse.
D - DISABILITY Check for disability.
- Check for any obvious disability/things that would prevent the patient from doing simple tasks (e.g. walking).
E - EXPOSURE Check for exposure.
- Check if any outside factors have affected the patient (e.g. electrocution).
(Similar to danger) Ask about the differences if you do not understand.
