A user joins the server and writes their introduction post. Then they got the Public Members role assigned and decided to spam the chat. Then proceeds to change their username to profanity words.

Answer:
Step 1: Immediately screenshot the spam and the audit logs where they changed their user.
Step 2: Proceed to implement a ban on a user to the bots and include Koko username, also let Koko speak to them in private.

You and some friends are in a voice channel, and a user joins the call. They are playing loud audio sounds, intentionally to hurt people's ears.
Answer:
Step 1: Immediately defenestration the user.
Step 2: Mute the user, and in the message, you include how long they're muted for and why they're muted.
(Note): If it's a first time offense, we can let the user know that they can still use the contact-us channel, to apologize for their behavior, and has the mute lifted. In addition, if they do apologize, and if the mute is lifted, and they continued the misconduct, then the mute will be twice as long as it was originally supposed to be.

A user messages you in the dm, and talks about how another user has made them feel uncomfortable. They proceed to explain how they used to play League of Legends together, but the last few times, the user flame and cursed at them in the voice call. Earlier in the day, the user started making fun of them to other users, even while they weren't playing.
Answer:
Step 1: Apologize to the user that they were made uncomfortable. And show them that we're investigating on this case.
Step 2: Contact the other user and let them know that it has been reported, that they were being toxic and that we are investigating this case to see what's going on. At this point, we asked them to provide us with a statement, explaining their side of the story. We would also contact the other users, that we're part of this situation. We would also collect statements from them as well.
Step 3: Write down an incident log, in the mod-logs chat channel, including the username of everyone involved; along with their user id. (Additional Note): Another possible option, depending on how things go, is to bring both for all users involved into the Conference chat channel for the Chamber of Secrets call channel, and act as a mediator to help find a peaceful resolution.
