Trachea C6 seviyesinde başlar ve mediastinum superiusta bulunur. T4 gövdesi alt kenar seviyesinde iki ana bronkus a ayrılır. Ayrıldığı bu bölgeye Bifurcatia Trachea (BT) denir. BT mediastinum medium dadır.
"Bifurcatio trachea, derin inspiryumda T6 seviyesine iner."
Carina trachea denilen çıkıntı ise bifurcatio trachea nın iç yüzünde sonuncu trakea halkasındadır. (2-4) trakea halkalarının önünde tiroid bezinin istimus u vardır.
Trachea veya soluk borusu, Th4-Th5 vertebra seviyesinde bronchus pricipalis dextra ve sinistra olmak üzere ikiye ayrılır.
Bronchus principalis dextra, sağ akciğer loblarına (lonus superior, medius ve inferior) giden bronchus lonaris superior, medius ve inferior dallarına ayrılır.
Bronchus principalis sinistra, sol akciğer loblarına (lobus superior ve inferior) giden bronchus lobaris superior ve inferior dallarına ayrılır.
Her bir bronchus lobaris, ulaştıkları segmentlere göre bronchıs segmentalis lere ayrılır. En küçük bronşlar, gaz alışverişinin yapıldığı saccus alveolaris lerle sonlanan bronşiollere ayrılırlar.
Şu kısım önemlidir ki:
Trakeotami veya trakeostomi de;
Yetişkinlerde: arcus venosus jugularis, v. thyroidea inferior ve (varsa) aç thyroidea ima,
Çocuklarda: timus ve v. brochiocephalica sinsitra
yaralanma riskinin yüksek olduüu bölgelerdir. Ve bu bölgeler gelen vakalarda iyi bir gözlem ile incelenmelidir ki her şey yolunda gitsin...
Son olarak Bronchus Principalis Sinister (BPS);
Arcus aortae nin altındam geçip, T6 seviyesinde hilum pulmonis e girer. Her bir tanesi 4 veya 5 adet segmental bronkus a ayrılan 2 lobar bronkus a ayrılır:
-- ilki Bronchus lobaris superior sinister
-- ikincisi bronchus lobaris imferior sinister
'dir.
Yapısal olarak BPS diğerlerine göre daha yatay, uzun ve çapı küçüktür.
Yine çok önemli bir kavram olan, yabancı cisimlerin en çok buraya alındığı Bronchus Principalis Dexter (BPD) V. azygos un aştından geçerek, T5 seviyesinde hilum pulmonis e girer. Ancak konuyu fazlaca aştığından ayrıntılı açıklamaya girmeyelim.
