Vaping and Electronic Cigarettes
E-cigarettes are not safe for youth, young adults, and pregnant women, and can be dangerous for adults who use tobacco products. Additionally, high quality epidemiology studies consistently demonstrate that e-cigarettes use increases conventional cigarette uptake, particularly among non-smoking youth, by nearly 3 times. Evidence reveals that these products are harmful to health and are not safe. However, it is too early to provide a clear answer on the long-term impact of using them or being exposed to them. Whilst long-term health effects are not fully known, we do know that they generate toxic substances, some of which are known to cause cancer and some that increase the risk of heart and lung disorders.

In a very recent study, chronic administration of nicotine to mice (1 mg/kg every 3 days for a 60-day period) enhanced brain metastasis by skewing the polarity of M2 microglia, which increases metastatic tumour growth [50]. Assuming that a conventional cigarette contains 0.172–1.702 mg of nicotine [51], the daily nicotine dose administered to these animals corresponds to 40–400 cigarettes for a 70 kg-adult, which is a dose of an extremely heavy smoker.

Study results showed the actual amount of the chemical was about 88% less than labeled. Their review gives us insight into the health effects and addictiveness of e-cigarettes and nicotine vapes. Based on the current literature, it seems that several factors have led to the success of e-cigarette use as a smoking cessation tool.

Exposure to emissions from e-cigarettes also poses risks to bystanders. All adults who smoke conventional cigarettes or other combustible (burned) tobacco products should be advised to quit smoking at the earliest opportunity, recognizing that quitting is hard and often takes repeated, dedicated efforts. Individuals can also seek cessation support by calling QUIT-NOW or ACS-2345. The study showed that the e-liquids of certain cig-a-like brands contain high levels of nickel and chromium, which may come from the nichrome heating coils of the vaporizing device.

The decision arrives as the FDA faces increasing criticism for failing to rein in illegal but widely available flavored vaping products that appeal to young people and comes amid mounting legal challenges over its handling of e-cigarette regulations. Endothelial cell migration is an important mechanism of vascular repair than can be disrupted in smokers due to endothelial dysfunction [25, 26]. Evidence demonstrates that e-cigarettes are harmful including for non-smokers, and especially for young people (1). However, products like cigarettes and smokeless tobacco continue to have a stronger hold in rural areas due to decades of targeted marketing by the tobacco industry, which integrated tobacco use into rural culture. In the first study, researchers looked at the impact of e-cigarettes on lipids and glucose in the blood. They recruited 476 healthy human participants without cardiovascular disease who were either nonsmokers, e-cigarette-only smokers, smokers of e-cigarettes and tobacco cigarettes and those who smoked tobacco cigarettes only.

Recent youth tobacco surveys found e-cigarette use had surpassed conventional cigarettes as the most commonly used tobacco product among youth. These surveys also revealed that many young people consider vaping to be safe and are not aware that e-cigarettes contain nicotine. It is easy for middle school and high school students to conceal their use of vaping devices because of their appearance as everyday objects.

Altria has told the FDA that they believe that some of the funding that they’ve been using on anti-vaping campaigns should also be used on teaching Americans that nicotine isn’t unhealthy.

Regulation of e-cigarettes varies widely across the world, from full bans (e.g. India); only being available on prescription (e.g. Australia); and to being freely sold as a consumer good (e.g. Europe, US). They already had sort of gone down that road, but a little bit reluctantly. The problem for Juul was, at the time, the FDA was cracking down on the industry. And they came out with this law that said, for the e-cigarette companies, that you could continue selling your product, but you couldn’t make any modifications to them. The FDA was essentially trying to freeze the market so there would be no new entrants and they could kind of wrap their arms around this unwieldy market.

CDC recommends you not use e-cigarettes or vapor products, especially those with THC. Fewer Pierce County youth are smoking cigarettes in recent years, but more are vaping. The main ingredient in vapes is propylene glycol, vegetable glycerine or glycerol, and almost all vapes contain nicotine, flavours and other chemicals. New national laws to strengthen controls on the importation, manufacture, and supply of all e-cigarette products are now in place.

And that ran right into some very strange regulatory moments that you unpack in the book. I’m curious if you can tell that story and kind of help me understand how they got through it. I’m really interested in that arc and sort of pulling apart the bits and pieces of that, but what struck me about your book is that you don’t start with Juul at all. You start with Altria, which is the company that was known as Philip Morris, that made Marlboro, and you start with them in sort of the very late ’90s, early 2000s. And it seems like what the entire cigarette industry was struggling with was, we’re going to be a regulated industry. So they did all kinds of just really organic research and ended up settling on benzoic acid as the organic acid that was the most satisfying.

Interestingly, there is a strong difference of opinion on e-cigarettes between countries. Whereas countries such as Brazil, Uruguay and India have banned the sale of e-cigarettes, others such as the United Kingdom support this device to quit smoking. The increasing number of adolescent users and reported deaths in the United States prompted the government to ban the sale of flavoured e-cigarettes in 2020. The difference in opinion worldwide may be due to different restrictions imposed.

Vaping works by heating liquid in a small device so you can breathe it into your lungs. The e-cigarette, vape pen or other vaping device heats the liquid in the device to create an aerosol. Mist from e-cigarettes contains particles of nicotine, flavoring and other substances suspended in air.

They all had different kinds of designs, and maybe they weren’t designed by Yves Behar or whoever, but the reason the Juul won was because the product was more effective at delivering nicotine. And by the way, these were advertising tactics that the tobacco industry was literally banned from engaging in. They hired people who knew nothing about tobacco to do their marketing and who knew the most about how to make a brand pop, how to make a product be super desirable. Reading your book, looking at the history of the research and how it’s politicized, it seems like the cigarette companies just created a lot of noise in the research.

E-cigarettes are battery-operated devices that convert liquid into aerosol, which is inhaled into the user’s lungs. Typically, the liquid contains addictive nicotine, as well as flavors. More than 10 million adults in the United States use e-cigarettes, and vaping has become especially popular among teenagers. While there appears to be some consensus that vaping may be less harmful to health than tobacco cigarette smoking, the dangers of e-cigarettes remain unclear. In the 2010s electronic cigarettes (aka e-cigarettes) and nicotine vapes exploded in popularity. People took interest in these products because they deliver nicotine through vapor instead of smoke.

Vaping may also have specific adverse effects on certain groups, particularly young people. Vaping appears to have a number of negative effects on oral health. The full magnitude of effects of toxic e-cigarette ingredients may not be known for another three decades. Interestingly, the existence of harmful concentrations of diethylene glycol, a known cytotoxic agent, in e-liquid aerosols is contentious with some studies detecting its presence [4, 68, 70,71,72], and others finding low subtoxic concentrations [73, 74]. Similar observations were reported for the content ethylene glycol. In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Glyoxal was detected in the method air blank and therefore the actual levels in the e-cigarette emissions are potentially lower than reported in Table 3. The concentration of nicotine in the BB57 emissions relative to BB18 was three times greater and followed the three-fold increase in nicotine strength of the e-liquid. In the cigarette smoke, the measured nicotine concentration of 0.23 ± 0.01 mg puff–1 (Table 1), equivalent to 2.07 ± 0.09 mg cig−1, is in accordance with the Ky1R6F certificate value of 1.9 ± 0.1 mg cig−1 (ISO Intense smoking regime)56.

Laboratory testing of seized illegal nicotine e-cigarette products has shown that e-cigarettes labelled 'nicotine-free' can have high nicotine levels. There is no single test for EVALI; it’s what is known as a “diagnosis of exclusion,” which means that a doctor will conduct tests to rule out other potential diseases and conditions. In the unadjusted analyses (Table 3), those planning for vocational education or extra year/discontinuation had higher S-EC compared with those planning for GUSS. Among girls, those without educational aspirations also had higher S-EC. Parental smoking, positive attitude towards e-cigarette use in one’s age group, current smoking, and snus use increased S-EC, whereas liking school lowered it. At school level, both among girls and boys, a higher proportion of students liking school and being never-users lowered S-EC, whereas a higher proportion of students with positive attitudes towards e-cigarette use increased it.

Further, it may influence the students’ descriptive and injunctive norms, which both predict smoking initiation among youth [27]. The most consistent predictors have been descriptive norms of parents’ and close friends’ smoking, indicating that addressing close network members might be the most effective in interventions [27]. In our study, parental smoking reported by adolescents increased all types of susceptibility when analysed independently.

Almost 9 in 10 people (86%) supported prohibiting the sale of e‑cigarettes to people under 18 years of age, a substantial increase from 2019 (79%). Social and economic factors shape people’s behaviours of vaping or smoking. Generally, people living in the most disadvantaged socioeconomic areas were the most likely to currently smoke regular cigarettes but not use e‑cigarettes (13.2%). In contrast, people living in the most advantaged socioeconomic areas were the most likely to use e‑cigarettes but not smoke regular cigarettes (6.6%) (Figure 4). In 2022–‍2023, 1 in 5 (19.8%) people aged 14 and over in Australia reported having used e‑cigarettes at least once in their lifetime, with 7% currently using e‑cigarettes at least daily, weekly, monthly, or less often than monthly.
