And it’s very much like what Adam and James talked about, the Juul co-founders, early on.

So the FDA is under huge pressure by the industry to go in this direction of harm reduction. And, of course, you have the anti-vaping people, the youth tobacco prevention people who want to see less of these products on the market because they continue to pose a threat to young people. It’s unavoidable that a bunch of public health mandates got deeply politicized and a lot of people in America are just like, “Don’t tell me what to do. I’m an adult, I’m going to take whatever risks I want to take, and that will be the end of it. Get out of my hair.” And those things seem like a collision course and the nexus of that collision is nicotine products.

I’m really interested in that arc and sort of pulling apart the bits and pieces of that, but what struck me about your book is that you don’t start with Juul at all. You start with Altria, which is the company that was known as Philip Morris, that made Marlboro, and you start with them in sort of the very late ’90s, early 2000s. And it seems like what the entire cigarette industry was struggling with was, we’re going to be a regulated industry. So they did all kinds of just really organic research and ended up settling on benzoic acid as the organic acid that was the most satisfying.

However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5]. In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46].

The viscosity and density of the e-liquid determine its mobility, capillary action, and delivery to the wick and coil, influencing the likelihood of a dry puff15. Sub-ohm vapes (sometimes called cloud kits) are more advanced vaping devices that are capable of outputting huge amounts of power; if you’ve ever noticed someone exhaling gigantic clouds from their kit, they were probably using one of these! Moreover, we just don’t know the long-term effects of vaping,” said Dr. Bhavna Barmi, a senior clinical psychologist.

In addition, the FTC found that there was a large shift in sales from tobacco-flavored e-cigarette products to fruit, candy, and dessert flavors. For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. Days before the Senate hearing, the FDA and federal law enforcement announced the creation of a task force to crack down on the flood of illegal e-cigarettes in vape shops and online marketplaces. The FDA also reversed a decision to ban products made by Juul Labs, the company widely cited for igniting youth vaping nearly a decade ago. Its products had remained on shelves as the company appealed the decision.

Vaping involves using a device known as an e-cigarette—also called a vape pen, mod, or tank—to heat up a small amount of liquid, turning it into a vapor that can be inhaled. Most vape liquids contain substances such as propylene glycol and glycerol as base ingredients that create the vapor.

However, initial studies have shown the vapor contains carcinogens. The quantity of these carcinogens is less that what is found in conventional cigarettes. Flavors, including mint and menthol, are one of the top reasons young people use e-cigarettes. Candy and fruit-flavored e-liquids can make e-cigarettes appealing and seem harmless. As of July 2020, the sale of flavored e-cigarettes is prohibited in NYC. Further, in 2018, one in 15 (6.7%) middle school students reported using e-cigarettes.

They examined coronary vascular function by a myocardial contrast echocardiography while participants were at rest and after performing a handgrip exercise to simulate physiologic stress. A recent study compared the acute effects of e-cigarette vapor (with PG/vegetable glycerine plus tobacco flavouring but without nicotine) generated from stainless‐steel atomizer (SS) heating element or from a nickel‐chromium alloy (NC) [92]. Neither the air‐exposed rats nor those exposed to e-cigarette vapor using SS heating elements developed respiratory distress. In contrast, 80% of the rats exposed to e-cigarette vapor using NC heating units developed clinical acute respiratory distress when a 70‐W power setting was employed. Thus, suggesting that operating units at higher than recommended settings can cause adverse effects.

The present study’s strength is that it is the first scoping review conducted to identify the environmental impacts of e-cigarettes. One limitation of our study is that only two databases were searched, which might have led to the absence of relevant studies. Manual screening of reference lists of publications included in the scoping review was used to add relevant publications that had not been initially identified through database searching.

This means that using e-cigarettes regularly can put you at risk to get infections like pneumonia or sepsis. To protect kids now, states and cities must also continue their growing efforts to end the sale of flavored e-cigarettes, as well as other flavored tobacco products. Battery-powered vape devices create an aerosol that looks like water vapor but contains nicotine, flavoring, and more than 30 other chemicals. The aerosol is inhaled into the lungs where the nicotine and chemicals cross over into the bloodstream.The earliest vape devices looked like cigarettes. Parents/caregivers, healthcare providers, and educators can influence the decision to use ENDS, like e-cigarettes/vapes.

In women smokers, a study found a significant difference in stiffness after smoking just one tobacco cigarette, but not after use of e-cigarettes (reviewed in [20]). Vapor products produce aerosol by heating a liquid solution, which is inhaled as an aerosol. The process is referred to as “vaping.” Vapor products often contain nicotine.

During a Senate Judiciary Committee hearing this month, Durbin and other senators ripped into the FDA over delays in reviewing company applications and over the proliferation of illegal flavored e-cigarette products, many manufactured in China. Durbin highlighted fruity and bubblegum-flavored vapes found in a shop about a mile from the FDA’s Maryland headquarters. Vapes come in colorful, fun, and unique product designs, along with many flavors, all of which come together as part of the attraction for youth who believe that these products are not addictive and are safe to use. Many users believe vaping will reduce stress, anxiety, and depression.

This group reported better physical health, exercised more and had more active social engagement, the study found. Reynolds Vapor Co. both sued the FDA after the agency ordered them to stop selling menthol vapes, a flavor popular with teens. In both cases, court-imposed stays halted the FDA’s orders pending review and the companies’ menthol products remain on the market. And in 2020, Trump backpedaled on his own plan to pull most flavored vapes off the market. Instead of banning all fruit and minty flavors, the Trump administration banned them only in “cartridge-based” devices such as Juul.

Nevertheless, there is no doubt that the deleterious effects of battery output voltage are not comparable to those exerted by CS extracts [30] (Figs. 1 and 2). This means that the Australian Therapeutic Goods Administration (TGA) has not assessed any e-cigarette products for quality, safety or performance as a smoking cessation aid. There are established pathways for consumers to legally access unapproved nicotine vaping products, with a valid prescription from a doctor. The FDA decision supports the vaping industry’s assertion that e-cigarettes can reduce the harms of smoking.

Here are some of the most common myths, and the facts based on scientific evidence and data. Nicotine itself is not very harmful and has been used safely for many years in medicines to help people stop smoking. E-liquids come in different nicotine strengths, so you control how much nicotine you need to help with cravings and other withdrawal symptoms, such as feeling irritable and having low mood.

The potential for long-term harm, coupled with its perceived attractiveness makes adolescent e-cigarette use a critical public health problem that requires urgent attention (Centers for Disease Control and Prevention [CDC], 2020a). Using e-cigarettes, or “vaping,” are terms used synonymously to refer to the use of a wide variety of electronic, battery-operated devices that aerosolize, but do not burn, liquids to release nicotine and other substances. Nicotine-containing e-cigarettes are regulated as “tobacco products” by the FDA because the nicotine is derived from the tobacco plant. E-cigarettes pose a threat to the health of users and the harms are becoming increasingly apparent. In the past few years, the use of these products has increased at an alarming rate among young people in significant part because the newest, re-engineered generation of e-cigarettes more effectively delivers large amounts of nicotine to the brain.

The majority of cases reported to poison centers are due to accidental exposures in children less than 6 years old. This reflects the increasing availability of these products in the home due to use by guardians or older siblings. Additionally, in the last 5 years, the FDA has warned companies selling e-cigarette devices made in the shape of toys, foods or cartoons that specifically appeal to use by young people. As e-cigarettes deliver flavorings, additives, and, typically, nicotine through an inhaled aerosol, they are harmful to both the user and those who are exposed secondhand. Surgeon General’s recommendation urging those who influence youth, young adults and people who are pregnant to discourage nicotine use in any form, including e-cigarettes. NSW Health has launched a new mass media behaviour change campaign aiming to address the issue of vaping by young people.

WHO regularly monitors and reviews the evidence on ENDS and health and offers guidance to governments. “Yale physicians have been at the forefront in identifying EVALI cases,” Dr. Weiss says. She explains that even before CDC guidelines on treating the illness were released, Yale doctors were successfully treating patients with a combination of antibiotics and steroids. Doctors at Yale Medicine also collaborate with researchers in the fields of tobacco and addiction medicine to provide care for patients with EVALI.

Much like free-base nicotine products, these liquids contain propylene glycol and/or vegetable glycerin, as well as other chemicals such as flavouring ingredients. The term "vaping" is also used to refer to the inhalation of cannabis aerosols, which are produced by products that differ from nicotine products described on this page. Since e-cigarettes arrived in the U.S. in 2007, they have been investigated by addiction researchers as possible cessation devices for adults trying to quit combustible, or regular, cigarettes. The FDA lists 93 harmful or potentially harmful chemicals found in regular cigarettes, and the National Cancer Institute (NCI) describes cigarettes as having more than 7,000 chemicals in them. Because e-cigarettes contain fewer chemicals the industry has presented them as a healthier alternative to regular cigarettes. But vape liquids can still contain nicotine, a highly addictive drug. E-Cigarettes, also called personal vaporizers or “vapes”, present another way for smokers to ingest nicotine.

In addition, the FDA and the Justice Department have obtained injunctions against six manufacturers to stop them from manufacturing and selling unauthorized e-cigarette products. Along with the FDA and the Justice Department, the task force will bring together multiple law enforcement partners, including the Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF); U.S. Additional agencies may join the task force in the coming weeks and months. In contrast to the clear evidence that flavored products fueled the youth e-cigarette epidemic, every major U.S. public health authority – including the U.S. Preventive Services Task Force, the CDC and even the FDA itself – has found there is inadequate evidence to conclude that e-cigarettes are effective at helping smokers quit. E-cigarettes, also known as e-cigs, vapes, vape pens, and electronic nicotine delivery systems (ENDS), are experiencing rapid growth in popularity, especially among teens and young adults.

Well, it still is, but no other e-cigarette on the market had 5 percent. One, because anything they put out is going to be immediately doubted as just being another study that’s been bought and paid for by the tobacco industry. It’s making it very hard for the company to come up with any credible science that people are actually going to take hook, line, and sinker. It feels like Juul had so much leverage because it had a product people liked better than cigarettes it was able to just extract, for itself, a cashout.

In addition, the VITA only heats its vape liquid to about 240 degrees Fahrenheit. That's much lower than many other kinds of vapes, so it won't be as rough on your lungs. LUVV also says their products are tested to "safeguard against" harmful byproducts of heating metal and vape fluid. They don't provide much detail to support this claim, though, so take it with a grain of salt.

"The amount of smoke the mice were exposed to was similar to a person who had vaped for about three to six years," they noted. One of the ingredients in tobacco is the mood-altering drug nicotine. Tobacco products contain unsafe substances, from acetone and tar to nicotine and carbon monoxide. The inhaled substances can affect your lungs and the other organs in your body. If you spend time around someone who vapes, you might be wondering if you can get secondhand smoke from a vape. While there's still a lot we don’t know about the harms of secondhand vaping, research suggests that bystanders who breathe in the aerosol might be exposed to many of the same toxins found in e-cigarettes and even some found in traditional tobacco.

There are also restrictions on liquids with flavors that may be more attractive to younger people. The researchers also noted a lack of evidence that vaping is an effective means of quitting smoking. The marketing of e-cigarettes and their range of flavors can give the impression that vaping is not harmful.

Young people who vape nicotine are more likely to start smoking cigarettes in the future. The 2018 NAP report concluded there’s some evidence that nicotine and nicotine-free e-cigarettes can damage oral cells and tissues in people who don’t smoke cigarettes. Finally, lung health effects are not expected to be seen for 20 to 30 years.

Without immediate measures to stop epidemic use of these products, the long-term adverse health effects will increase. Despite their popularity, little is known about their health effects. Some suggest that e-cigarettes likely confer lower risk compared to combustible tobacco cigarettes, because they do not expose users to toxicants produced through combustion. Proponents of e-cigarette use also tout the potential benefits of e-cigarettes as devices that could help combustible tobacco cigarette smokers to quit and thereby reduce tobacco-related health risks.

They all had different kinds of designs, and maybe they weren’t designed by Yves Behar or whoever, but the reason the Juul won was because the product was more effective at delivering nicotine. And by the way, these were advertising tactics that the tobacco industry was literally banned from engaging in. They hired people who knew nothing about tobacco to do their marketing and who knew the most about how to make a brand pop, how to make a product be super desirable. Reading your book, looking at the history of the research and how it’s politicized, it seems like the cigarette companies just created a lot of noise in the research.

Among girls, a higher proportion of girls in the school, students with a high parental education and planning for GUSS lowered S-EC, but a higher proportion of students with parental smoking increased it. Carbonyls in cigarette smoke are formed mainly by pyrolysis of tobacco sugars83, whereas those in e-cigarettes are formed mainly by thermal degradation of PG and/or VG83,84,85. Flavourings may also contribute to the formation of carbonyls, as well as the characteristics of the e-cigarette devices, especially the applied voltage, coil resistance and wicking material47,48,49,86,87. Poor wicking efficiency may lead to a dry wick and overheated e-liquid (dry puff), which promotes the formation of carbonyls and other toxic compounds2,10,13,15. Coil location, orientation, resistance and wick material, as well as power output, have been shown to affect carbonyl generation significantly13,15,86. E-liquid physical properties are also important in carbonyl formation15,47,84,86.

While e-cigarette use prevalence has declined significantly since the heyday of JUUL — 10% of high school students reported using e-cigarettes in 2023 — e-cigarette use among young people remains a concern. Thousands of flavored, high-nicotine, and relatively cheap e-cigarette products remain on the market — many of them illegally — driving youth use and nicotine addiction. Equally concerning, nearly half of young people who have ever tried e-cigarettes continue to use them, and many do so daily. Although e-cigarettes have been sold in the U.S. for nearly 20 years, use patterns have shifted dramatically. As newer iterations brought higher levels of nicotine in an increasing array of flavors and product designs, youth use skyrocketed.

Overall, recent figures from JAMA suggest more than 2million kids under 18 vape regularly, along with 17million adults. Smoking can raise your risk for periodontal disease or disease affecting the gums. This happens because smoking causes inflammation around the teeth and increases your risk for bacterial infections.

This study explored the prevalence of S-SM, S-EC, and S-SN among adolescents in a country with advanced tobacco control regulations. We found that the susceptibility was highest for e-cigarette use, followed by smoking and snus use. S-SM was more common among girls, whereas S-EC and S-SN were more common among boys.

Similar increases were seen in the sale of candy and dessert-flavored cartridges, as well as the sale of disposable e-cigarettes with these flavors. The data collected show a dramatic increase in e-cigarette sales in general between 2015 and 2018 (from $304.2 million to $2.06 billion, respectively), as well as a similarly dramatic increase in the sales of cartridge e-cigarette systems such as Juul’s. The companies sold $260 million worth of such cartridge systems in 2015, with sales growing to $1.969 billion in just three years.

Symptoms include vomiting, rapid heart rate, unsteadiness and increased salivation. The long-term effects of smoking cigarettes are well-documented and include an increased risk of stroke, heart disease, and lung cancer. Recently, the FDA announced that it has made decisions on 99% of e-cigarette applications, noting that it had rejected millions and authorized 23. The composition of e-liquids requires stricter regulation, as they can be easily bought online and many incidences of mislabelling have been detected, which can seriously affect consumers’ health.

This legislation, which applies to all states, raises the minimum age of sale for all tobacco products— including e-cigarette—to 21. The American Lung Association is very concerned that we are at risk of losing another generation to tobacco-caused diseases as the result of e-cigarettes. The Lung Association remains extremely troubled about the rapid increase of youth using these products and has repeatedly called upon the Food and Drug Administration (FDA) to increase their oversight and scrutiny of these products to protect kids. Truth Initiative is America's largest nonprofit public health organization committed to preventing youth and young adult nicotine addiction and empowering quitting for all. That's hard to say — the potential health risks of vaping CBD are still hotly debated.

Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47]. In this latter regard, no differences were encountered in the frequency of tumour appearance in rats subjected to long-term (2 years) inhalation of nicotine when compared with control rats [48]. Despite the lack of carcinogenicity evidence, it has been reported that nicotine promotes tumour cell survival by decreasing apoptosis and increasing proliferation [49], indicating that it may work as a “tumour enhancer”. In a very recent study, chronic administration of nicotine to mice (1 mg/kg every 3 days for a 60-day period) enhanced brain metastasis by skewing the polarity of M2 microglia, which increases metastatic tumour growth [50]. Assuming that a conventional cigarette contains 0.172–1.702 mg of nicotine [51], the daily nicotine dose administered to these animals corresponds to 40–400 cigarettes for a 70 kg-adult, which is a dose of an extremely heavy smoker.

“The PACT Act is an important tool for preventing the unlawful sale of e-cigarettes to minors online. ATF looks forward to working with other components of the Department of Justice and USPIS to enforce the law,” said ATF Director Steven M. Dettelbach. According to both the Center for Disease Control and Prevention and the United States Surgeon General, e-cigarette use among young people is unsafe.

I think immediately the media attention gravitated toward Juul because Juul was the most popular product on the market. Juul had been singled out as the one causing the youth vaping epidemic. And now suddenly you have all these kids who vaped showing up at the emergency rooms.

They were smoking cigarettes out back as they kind of pondered their next project, and they realized that it was idiotic to be smoking this burning stick, essentially shredded tobacco leaf rolled in paper, lit on fire. The same product that people had been using for more than a century. Levels of naphthalene were slightly higher than those of pyrene in e-cigarette emissions, while the respective air blank was lower. Nevertheless, it seems likely that these compounds were detected in e-cigarette emissions due to their presence as low-level contaminants in the background air, rather than originating from the vaping product. Today, nicotine poisoning has become a growing concern because of new nicotine products on the market, particularly electronic cigarettes (e-cigarettes) and pure liquid nicotine. The concentration of nicotine in liquid products is higher than most other tobacco products.

It is not yet known if this has negative effects on the body, but Sven-Eric Jordt, PhD, one of the study’s authors, says he hopes that the FDA will begin to study the short- and long-term effects of inhaled acetals. Deepa Camenga, MD, a pediatrician who is board-certified in addiction medicine, says it’s never too early to begin talking about e-cigarettes in age-appropriate language. “When you are out and about with your children and see an advertisement, for example, take the opportunity to talk about it,” she says. As they grow older, parents can expand on their thoughts and expectations. “It’s also important to give teens and young adults the space to ask questions,” she says. Njoy’s products accounted for less than 3% of U.S. e-cigarette sales in the past year, according to retail data from Nielsen.

Reynolds’ Velo nicotine pouches, while holding just a 1.2% market share, has had a 25.8% year over year sales growth, as well as up 96.7% for the four-week period. Nicotine pouches have a 5.1% market share with sales up 73.4% over the year. Tobacco industry executives and analysts are sounding alarms about a surge of unregulated nicotine pouches entering the U.S. that is chipping away at regulated products. This includes data on youth use of these products, which has led to development of several educational programs designed to prevent adolescents and teens from using these products. Through tobacco product problem reports and tobacco product violation reports, the FDA also knows much more about many safety and health hazards they may pose. The Surgeon General reports e-cigarette use among youth is a significant public health concern and steps must be taken by parents, educators and especially policymakers to discourage use of e-cigarettes.

E-cigarettes are battery-operated devices that can look like a real cigarette or pen. There are hundreds of brands, and they're sometimes marketed as a way to get your nicotine fix without the danger of cigarettes. “This research has provided greater understanding in the court systems nationwide as to the nature of drug usage, abuse, and overdose cases in which electronic cigarettes were used to deliver an illicit drug,” the researchers concluded. Many of those who vape were likely to report issues that kept them from being able to safely dispose of their used and empty e-cigarette products. In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Figure 1 lists the main compounds detected in aerosols derived from humectant heating and their potential damaging effects.

In the multivariate models, most of the school-level associations were weakened. Among girls, only a higher proportion of never-users in the school remained protective for S-SM and S-EC. Among boys, the same was observed for S-EC, whereas a higher proportion of students with positive attitudes towards snus use in one’s age group increased S-SN. Interestingly, a higher proportion of students planning for general upper secondary education had lowered S-EC and S-SN in the univariate analyses but increased S-SM and S-SN in the multivariate models. This may reflect, for instance, more complex interactions between individual- and school-level factors influencing susceptibility among boys. In general, students with positive attitude towards product use in one’s age group and current use of some other tobacco or nicotine product had consistently higher susceptibility regardless of product type.

The prognosis depends on how much nicotine was taken and how quickly treatment was started. If a person is able to survive during the first four hours after poisoning, they’re usually likely to recover. Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]). In contrast, other studies found acute microvascular endothelial dysfunction, increased oxidative stress and arterial stiffness in smokers after exposure to e-cigarettes with nicotine, but not after e-cigarettes without nicotine (reviewed in [20]).

While this should reduce the number of flavored cartridges available in the United States, the guidelines are limited to products submitted for market authorization and don’t include those made at home or purchased online from other countries.
