Didanosine-induced Retinopathy: New Experience together with Long-term Follow-up.
Median time to recognition of the need for CPR for calls in which the chief complaint description was pursued was 166 s compared to 62 s for calls in which the chief complaint description was not pursued. CONCLUSION Caller chief complaint description affects the time to recognition of the need for T-CPR. CLINICAL TRIAL REGISTRATION http//www.clinicaltrials.gov Trial # NCT01972087. V.AIM The mathematical method used to calculate chest compression (CC) rate during cardiopulmonary resuscitation varies in the literature and across device manufacturers. The objective of this study was to determine the variability in calculated CC rates by applying four published methods to the same dataset. Penicillin-Streptomycin chemical structure METHODS This study was a secondary investigation of the first 200 pediatric cardiac arrest events with invasive arterial line waveform data in the ICU-RESUScitation Project (NCT02837497). Instantaneous CC rates were calculated during periods of uninterrupted CCs. The defined minimum interruption length affects rate calculation (e.g., if an interruption is defined as a break in CCs ≥ 2 s, the lowest possible calculated rate is 30 CCs/min). Average rates were calculated by four methods 1) rate with an interruption defined as ≥ 1 s; 2) interruption ≥ 2 s; 3) interruption ≥ 3 s; 4) method #3 excluding top and bottom quartiles of calculated rates. American Heart Association Guideline-compliant rate was defined as 100-120 CCs/min. A clinically important change was defined as ±5 CCs/min. The percentage of events and epochs (30 s periods) that changed Guideline-compliant status was calculated. RESULTS Across calculation methods, mean CC rates (118.7-119.5/min) were similar. Comparing all methods, 14 events (7%) and 114 epochs (6%) changed Guideline-compliant status. CONCLUSION Using four published methods for calculating CC rate, average rates were similar, but 7% of events changed Guideline-compliant status. These data suggest that a uniform calculation method (interruption ≥ 1 s) should be adopted to decrease variability in resuscitation science. The error-related negativity (ERN) is an event-related potential occurring in the electroencephalogram (EEG) within 100 ms after the commission of an error. The ERN is thought to partially reflect emotionally aversive aspects of error commission, however, it has thus far not been related to the neural processing of other aversive events, such as brief aversive bodily sensations. Therefore, the present study investigated the links between the ERN and the N1 amplitudes of respiratory-related evoked potentials (RREP) and somatosensory evoked potentials (SEP). During the acquisition of high-density EEG, 41 healthy participants performed a Flanker task to evoke the ERN, while RREP and SEP were separately elicited, using inspiratory occlusions and electrocutaneous stimulation of the wrist. Significant positive correlations were observed between the amplitudes of the ERN and the N1 of RREP and SEP, suggesting relationships between the neural processing of different emotionally aversive events, namely errors and bodily sensations. Mixtures of substances to which humans are exposed may lead to cumulative exposure and health effects. To study their effects, it is first necessary to identify a cumulative assessment group (CAG) of substances for risk assessment or hazard testing. Excluding substances from consideration before there is sufficient evidence may underestimate the risk. Conversely, including everything and treating the inevitable uncertainties using conservative assumptions is inefficient and may overestimate the risk, with an unknown level of protection. An efficient, transparent strategy is described to retain a large group, quantifying the uncertainty of group membership and other uncertainties. Iterative refinement of the CAG then focuses on adding information for the substances with high probability of contributing significantly to the risk. Probabilities can be estimated using expert opinion or derived from data on substance properties. An example is presented with 100 pesticides, in which the retain step identified a single substance to target refinement. Using an updated hazard characterisation for this substance reduced the mean exposure estimate from 0.43 to 0.28 μg kg-bw-1 day-1 and reduced the 99.99th percentile exposure from 24.9 to 5.1 μg kg-bw-1 day-1. Other retained substances contributed little to the risk estimates, even after accounting for uncertainty. In food and toxicology science, a huge amount of research and other data has been collected. To enable its full utilization, advanced statistical and computer methods are required. All data is related to food items, but additionally include different kinds of information. Nowadays the consumption of avocado has increased. To understand the full impact of this increased consumption on public health and the environment, different data related to avocado need to be considered. In this paper, we present an approach for representing foods in the form of vectors of continuous numbers (food embeddings) as an alternative solution to manual indexing. The utility of representing food data as a vector of continuous numbers was evaluated and demonstrated in four tasks i) automated determination of different food groups, ii) automated detection of the food class for each food concept (raw, derivative or composite), iii) identification of most similar food concepts for a given food concept, and iv) qualitative evaluation by a food expert. The experimental results showed that these kind of vector representations outperform the traditional representational methods used for food data analysis, and thus they present a step forward to more advanced food data analysis used for discovering new knowledge. Combination therapy is an alternative approach to reduce viral shedding and improve clinical outcomes following influenza virus infections. In this study we used oseltamivir (OST), a neuraminidase inhibitor and nitazoxanide (NTZ), a host directed drug, and found in vitro that the combination of these two antivirals have a synergistic relationship. Using the ferret model of (A/Perth/265/2009, (H1N1)pdm09), virus infections, we found that the combination of NTZ and OST was more effective than either NTZ or OST independently in preventing infection and reducing duration of viral shedding. However, these benefits were only seen if treatment was administered prophylactically, as opposed to therapeutically. We also found that if prophylactically treated ferrets that had detectable virus in the upper respiratory tract, no virus was detected in the lower respiratory tract. This benefit was not observed with NTZ or OST alone. The combination of NTZ and OST enhances the antiviral effect of OST, which is the standard of care in most settings. V.Status epilepticus (SE) is an emergency condition that can cause permanent brain damage or even death when generalized convulsive seizures last longer than 30 min. Controlling the escalation and propagation of seizures quickly and properly is crucial to the prevention of irreversible neuronal death and the associated morbidity. However, SE often becomes refractory to current anticonvulsant medications, which primarily act on ion channels and commonly impose undesired effects. Identifying new molecular targets for SE might lead to adjunctive treatments that can be delivered even when SE is well established. Recent preclinical studies suggest that prostaglandin E2 (PGE2) is an essential inflammatory mediator for the brain injury and morbidity following prolonged seizures via activating four G protein-coupled receptors, namely, EP1-EP4. Given that EP2 receptor activation has been identified as a common culprit in several inflammation-associated neurological conditions, such as strokes and neurodegenerative diseases, selective small-molecule antagonists targeting EP2 have been recently developed and utilized to suppress PGE2-mediated neuroinflammation. Transient inhibition of the EP2 receptor by these bioavailable and brain-permeable antagonists consistently showed marked anti-inflammatory and neuroprotective effects in several rodent models of SE yet had no noticeable effect on seizures per se. This review provides overviews and perspectives of the EP2 receptor as an emerging target for adjunctive treatment, together with the current first-line anti-seizure drugs, to prevent acute brain inflammation and damage following SE. ETHNOPHAMACOLOGICAL RELEVANCE The Chinese herbal prescription JieZe-1 (JZ-1) is based on the modification of Yihuang Tang, which was first described in Fu Qingzhu Nvke by the famous Qing Dynasty doctor Shan Fu as a treatment for leukorrheal diseases. As an in-hospital preparation, JZ-1 has been used in Tongji Hospital for many years to treat various infectious diseases of the lower female genital tract, including cervicitis, vaginitis, genital herpes and condyloma acuminatum. Our previous studies have shown that JZ-1 has curative effects on Candida albicans, Trichomonas vaginalis and Ureaplasma urealyticum infections. AIM OF THE STUDY Genital herpes is among the most common sexually transmitted diseases (STDs) worldwide and is mainly caused by herpes simplex virus type-2 (HSV-2). Current therapies can relieve symptoms in patients but do not cure or prevent the spread of the virus. This study was designed to investigate the effect of JZ-1 on HSV-2 infection and its mechanism, which is based on autophagy inductSV-2 infection may result in the blocked autophagic flux and inhibited autophagosome and autolysosome formation. JZ-1 exhibited significant antiviral activity in the VK2/E6E7 cells, which showed increased cell vitality and reduced viral protein expression, namely, earliest virus-specific infected cell polypeptides 5 (ICP5) and glycoprotein D (gD). We found that JZ-1 treatment inhibited the upregulation of the PI3K/Akt/mTOR pathway proteins and promoted autophagy to combat HSV-2 infection, while PI3K inhibitor pretreatment prevented the enhanced autophagy induced by JZ-1. Moreover, JZ-1 attenuated the increase in inflammatory cytokines that had been induced HSV-2 infection. CONCLUSION Our results showed that JZ-1 protects against HSV-2 infection, and this beneficial effect may be mediated by inducing autophagy via inhibition of the PI3K/Akt/mTOR signaling axis. V.ETHNOPHARMACOLOGICAL RELEVANCE Garlic and its wild relatives have a long tradition of use world-wide dating back to the early ages as food and medicine. This exceptional group deserves to be taken into consideration to rethink its potential health effects in light of its cultural and historical background, as well as ethnobotanical practices. AIM OF THE STUDY Garlic, onion, and other related vegetables have been indispensable for Turkish daily life. This review focuses on the wild, as well as cultivated, Allium species from the broad historical perspective of Turkish culture. Its goal is also analyze how food and medicine have been perceived as closely connected entities in a specific cultural sphere. METHODS A comprehensive literature survey was conducted on these species by referencing scientific databases such as Google Scholar, Science Direct, PubMed, Elsevier E-Book, Scopus, ISI-Web of Science, and E book Central (Ebrary). RESULTS The integration of historical and ethnobotanical data have enabled us to understand the unique position of Alliums in a specific human geography.
