In this work, a CNN architecture, StepNet, with step-wise incremental convolution layers for each downsampled block was developed for classification of breast tumors as benign/malignant. To increase noise robustness and as an improvement over existing methodologies, neutrosophic preprocessing was performed, and the enhanced images were appended to the original image during training and data augmentation. The final layers' activation maps are clustered using fuzzy c-means clustering which qualify as a validation method for the prediction of StepNet. Using neutrosophic preprocessing alone had increased the validation accuracy from 0.84 to 0.93, while using neutrosophic preprocessing and augmentation had increased the accuracy to 0.98. StepNet has comparably less training and validation time than other state of the art architectures and methods and shows an increase in prediction accuracy even for challenging isoechoic and hypoechoic tumors.Emerging evidence suggests that vicarious racial experiences of discrimination may negatively influence child health. Few studies have focus on childhood asthma symptoms and potential moderators of such relationship.
We used two population-based cross-sectional studies from the Social Change Allergy and Asthma in Latin America project in Salvador, Brazil. A total of 1003 children and mothers interviewed in 2006 were included, of whom 873 were reached again in 2013. Vicarious racial discrimination was assessed in mothers by applying the Experiences of Discrimination scale. Data on wheeze and environmental exposures were collected with standardized questionnaires. Levels of allergen-specific IgE were measured to identify atopy. Generalized estimating equations were used to estimate the association between maternal discrimination and wheezing and asthma phenotypes. Interaction terms were evaluated to identify whether mothers' mental health and family social support modified such associations.

Children whose mothers reported racial discrimination had greater odds of have asthma symptoms (OR 1.75; 95% CI 1.15-2.67) and non-atopic asthma (OR 1.92; 95% CI 1.09-3.40). When we considered effect modification by social support, we found a higher ORs when the level of social support was lower (OR 2.43; 95% IC 1.19-4.97) than when the level of social support was higher (OR 1.12; CI 0.64-1.96).

Transradial access (TRA) has become the primary route for coronary angiography (CAG) and percutaneous coronary interventions (PCI). Recently a new puncture site more distally in the area of the anatomical snuffbox has been described. With this multicenter registry, we wish to demonstrate the feasibility and safety of the distal radial access (dRA).

Between December 2018 and May 2019 all patients with a planned CAG or PCI via dRA in three cardiology centers in Germany were entered into this registry. Procedural data, puncture success, crossover rate and complications were registered. Proximal and distal radial artery patency were examined by ultrasound within 48h.

A total of 327 patients were enrolled (mean age 69 ± 12years, 69% male gender, 49% PCI), in 5 cases bilateral distal puncture was performed. Puncture success, defined as completed sheath placement was high (N = 316/332, 95%) and the crossover rate was low (27/332, 8%). The rate of proximal radial artery occlusion after 1-48h was low (2/332 1%), the rate of occlusion at the distal puncture site was also very low (3/332, 1%). Major complications were not encountered.

Coronary angiography and interventions via the distal transradial access in the area of the anatomical snuffbox can be performed with a high rate of success and safety. This data suggests a reduced rate of radial artery occlusion compared to previously reported data after cannulation via the standard forearm radial artery puncture site. Randomized studies are needed to further investigate these results.

This volume is organized thematically into six topical domains that parallel the major subspecialty areas of contemporary clinical sleep neurology practice, as well as novel directions and opportunities. The issue begins with an overview of the central disorders of hypersomnolence, including narcolepsy, idiopathic hypersomnia and other hypersomnia disorders, and the related use of the entire broad range of stimulant and wake-promoting pharmacotherapies. Next, the range of behavioral therapies, application of light and light restriction and melatonin therapies, and hypnotic pharmacotherapies useful in insomnia and circadian sleep-wake rhythm disorders are reviewed, followed by an overview of treatment options for sleep-related breathing disorders including positive airway pressure and the novel approach of hypoglossal neurostimulation for obstructive sleep apnea. The parasomnias and sleep-related movement disorders, including NREM disorders of arousal, REM parasomnias (nightmares and isolated sleep paralysis and idiopathic/isolated REM sleep behavior disorder, and restless legs syndrome are then discussed, and the applications of sleep neurotherapeutics in sleep and neurological disease are reviewed, including neurodevelopmental, epileptic, autoimmune encephalopathies, and neurodegenerative diseases. Last, the novel directions and opportunities in sleep neurology offered by cannabinoid therapies and machine learning/artificial intelligence methodology conclude this comprehensive survey of contemporary sleep neurology. It commonly co-occurs with psychiatric and medical conditions, which may further exacerbate these comorbid conditions and hinder treatment response. There is much empirical evidence to support the clinical efficacy of non-pharmacological treatment for insomnia, especially cognitive behavioral therapy for insomnia (CBT-I), in managing insomnia in a wide range of populations. This article reviews the research on the efficacy of CBT-I for primary insomnia and insomnia comorbid with other psychiatric and medical conditions, the empirical evidence regarding different CBT-I treatment modalities, the implementation of CBT-I across different age groups, and some initial evidence on the sequential combination of insomnia treatments. A brief overview of other non-pharmacological treatment with regard to complementary alternative medicine is also provided.

The difficulties encountered during the machining of this type of material are linked to the low density of Nomex paper, and to the low thickness of the walls forming the honeycomb cells of this type of structure. In this work, a finite element calculation code "ABAQUS`- EXPLICIT" was used to optimize and analyze the machining by milling of Nomex honeycomb structures. The main objective of this work is to study the influence of the machining conditions on the cutting forces, and the morphology of the chips.In North Africa, barley (Hordeum vulgare L) is the second most cultivated cereal. In Tunisia, barley is cultivated in mining areas with possible Cd soil contamination. The accumulation of Cd was studied in the 36 most cultivated North African barley cultivars cultured during 6 months on control soil and on soil containing 10 ppm of Cd. Cadmium did not affect germination and morphology in any cultivar. However, Cd induced variable effects on the biomass according to the cultivar. The cultivar Lemsi was the most sensitive one and Gisa 127 the most tolerant to Cd. The spike morphology did not show any differences between control and Cd-treated plants. The number of grains per spike and the weight of kernels were differently affected by Cd. On this basis, we identified Manel, Temassine, Giza 130, and Firdaws as the most tolerant cultivars and Raihane, Giza 123, Adrar, and Amira as the most sensitive ones. Cd accumulated at a higher concentration in straw than in the grains, but for both organs, we observed a significant intraspecific variability.
