Advanced practice providers (APPs), including nurse practitioners (NPs) and physician assistants (PAs), are part of a growing cancer care workforce. Current hematology-specific education provided by most graduate NP and PA school educations is limited. Mayo Clinic School of Health Sciences launched a hematology-specific fellowship in 2018 to provide APPs with the skills and knowledge required to deliver high-quality specialty care in hematology and blood and marrow transplant (BMT). The fellowship curriculum was developed based on a needs-based assessment study as well as the qualitative reported experiences of current hematology-specific APPs. The curriculum contains didactic in-class education, research opportunities, and mentored clinical rotations in both inpatient and outpatient practice in hematology and BMT. This 12-month fellowship, one of the only postgraduate training programs dedicated to benign and malignant hematology practice, provides structured training for highly qualified graduate APPs interested in developing a rewarding career in hematology.Waldenström macroglobulinemia is a rare hematologic malignancy characterized by an IgM-associated lymphoplasmacytic lymphoma. Often, it is associated with an indolent disease course, and many patients are candidates for careful monitoring. As many patients present with advanced age and nonspecific constitutional symptoms, careful consideration should be given to treatment decisions, including when and how to treat for maximized clinical benefit with minimal toxicity. This article provides an evidence-based practical approach to appropriate monitoring of the asymptomatic patient and management of symptomatic patients who require treatment for this rare malignancy.Patients receiving ifosfamide as part of their cancer treatment are at risk for ifosfamide-related encephalopathy (IRE), a potentially serious adverse event affecting up to 60% of patients. Symptoms range from transient altered mental status to coma and death. Consensus regarding risk factors for the development of IRE has not been reached in the literature. The purpose of this review is to identify risk factors for the development of IRE in adult cancer patients. A literature review was completed by searching PubMed and Scopus databases to identify articles published between 2008 and 2018. A total of 76 search results were reduced to a final sample of seven articles after applying inclusion and exclusion criteria. Published data suggest that Eastern Cooperative Oncology Group (ECOG) performance status of greater than or equal to 2, impaired renal function, hypoalbuminemia, and having multiple risk factors are risk factors for the development of IRE. Knowledge of which patients are at increased risk for the development of IRE could help clinicians to appropriately counsel patients and families regarding personal risk for the development of IRE. Clinicians may also more closely monitor patients with risk factors for IRE.
The number of newly approved cancer medications continues to grow; many of these newly approved medications are oral agents. Oral oncolytic agents have advantages including patient convenience, prolonged drug exposure, and noninvasive administration. However, these advantages come at a cost premium that many patients cannot afford, which can lead to change in therapy or abandonment. This study evaluates the perceptions of health-care workers regarding the cost and safety of oral oncolytic agents.

This is a descriptive, nonexperimental, cross-sectional study of health-care professionals in hematology/oncology patient care settings across the United States. Data were collected using a 35-item online questionnaire to measure quality improvement areas when using oral oncolytic agents.

Results are based on 503 survey respondents comprising mainly pharmacists (54%), pharmacy administrators (15%), and nurses (10%). Adherence to oral oncolytics was not included in outcome measurements at 31.5% of respondents' fmedication. Until recently, cancer therapy consisted primarily of IV infusion therapy, but oral oncolytic agents have been added to the therapy options over the past several years.Acute myeloid leukemia (AML) is a disease of older adults, with a median age at diagnosis of 68 years. The availability of oral anticancer medications has increased, although the standard treatment for AML remains in intravenous form. We aim to identify barriers to adherence to oral medications in patients with AML and proposed solutions for improvements. Following institutional review board approval, patients with AML and their caregivers were recruited to participate in focus groups. Sessions were digitally recorded, transcribed verbatim, and analyzed for thematic content using Dedoose qualitative software. 11 patients (five less then 65 years; six ≥ 65 years) and 4 caregivers participated in these sessions. Three central themes emerged 1) medication adherence challenges, 2) managing an oral adherence plan, and 3) strategies to improve oral adherence. Participants recommended written schedules, taking medications around meals, and using pillboxes and alarms. We believe that patients are an important source of insight into barriers and solutions to oral medication adherence.Immune thrombocytopenia (ITP) is a disorder characterized by immune-mediated destruction of thrombocytes leading to peripheral blood platelet count of less then 100 × 10^9/L. Primary ITP is a terminology used in the absence of other causes or disorders that may be associated with thrombocytopenia, i.e., isolated thrombocytopenia. The term secondary ITP is used if such diseases coexist. We present here a case of a 14-year-old female diagnosed with immune thrombocytopenia. Contrary to the popular belief about secondary ITP in adult population, treatment of our patient's hypothyroidism did not improve her platelet's count, and the patient needed multiple immunosuppressive medications to improve her condition.
