Şeklinde dilekçe yazarak okul muavinine verebilirsiniz. Ayrıca eğer veli de çalışıyor/hastalık gibi bir konu ise okula gelip vermesine gerek yok bu dilekçeyi doldurup öğrencisinden okula gönderebilir, ya da başka zaman gelip kendi de teslim edebilir. Eğer dilekçeyi hazır boş olarak düzenli bir şekilde indirmek istiyorsanız lütfen alt kısma bakınız.
