F55 presents to the ED c/o SOB x 3 days. Admits N/V/D, productive cough, pressure in ears bilaterally, dizziness upon turning her head to the left, and chest and head congestion. Denies chest pain. Pt states SOB is exacerbated by walking and coughing. Pt states SOB is relieved by laying down Pt states her inhaler was not working. Pt states she had a bowel accident on her way to the ED and states her stool is softer than normal. Pt states she quit smoking 2.5 years ago. Pt denies hx of congestive heart failure. Allergies include Lorazepam.

M39 presents to the ED c/o burning epigastric pain which radiates to the periumbilical and RLQ. Admits constipation and vomiting. Denies RUQ pain. Pt states he has seen a GI specialist in the past few weeks to have a GI scope. Pt states he was told he had gastritis and GERD. Pt states he has been using Protonix and Pepsid to mitigate his symptoms but has not been following the dietary advice he was given. Pt admits he has been smoking and eating unhealthily since his diagnosis. Pt states pain is sharp and exacerbated by chewing and swallowing. Pt denies hx of hyperlipidemia, DM, and cardiac conditions. Pt states his mother has hx of DM but not of cardiac conditions. Allergies include Codeine Sulfate and Ultram.

F78 presents to the ED c/o a UTI. Admits polyuria and dysuria. Pt states she regularly sees a urologist. Pt states she presented to the ED 2 weeks ago and was prescribed Macrobid, which she states has not been effective. Pt states she previously used Cipro, which was effective. Medical hx includes HTN and Takotsubo Cardiomyopathy. Allergies include Anesthesia Stopcock Set, Codeine Sulfate, Morphine, Nitroglycerin, Penicillins (rash, non anaphylaxis, and no swelling), and Sulfur.

F49 presents to the ED with worsening right rib pain under the right breast around ribs 6 and 7 radiating down her right side. Admits cough. Denies any back pain. Pain is exacerbated by raising her right arm, coughing, and taking deep breaths. Pt denies long travel and smoking. Pt states she socially drinks very rarely. Medical hx includes Crohn's Disease. Allergies include Codeine Sulfate, E-Mycin, KeFLEX, NSAIDS, and Penicillins.

F18 presents to the ED with mom c/o posterior neck pain following a MVA. Denies LOC and memory loss. Upon palpation of the C-spine, the Pt commented that it "felt good", and the mother laughed. Pt states she was sitting in the left rear seat with her seatbelt on when the car she was travelling in was rear ended. The airbags in the car did not deploy, and the Pt was able to ambulate following the accident. Pt has no other medical complaints at this time. NKDA.

F51 presents to the ED c/o vaginal bleeding x 4 days following sexual intercourse. Admits pain when cleaning the area with wet wipes. Pt states she developed a gash in her vagina from rough intercourse. While undergoing oral sex afterwards, Pt states she thinks her partner bit her. Pt states she was sent by her OBGYN.



F49 presents to the ED c/o migraine. Admits . Denies . Pt states . Medical hx . Surgical hx . Allergies .
